Nine times out of ten it is the turning chain, either counted as a stitch when it should not be, or skipped when it should. Check the note under Turning chains on page two of the pattern, then recount the row below. If it still does not add up, send me a photo.
Almost always. Match the yarn weight and check your gauge swatch against the pattern square. If it matches, you are fine. Premium patterns include a substitution chart with tested alternatives at three price points.
Custom pattern design, yes, from $480 with a 4 to 6 week turnaround. I do not sell finished items, but yarn shops can licence patterns as printed kit inserts. Use the form and mention wholesale in the message.
